RESUMO

Disability is a dynamic process and can be influenced by a sociocultural environment. This study aimed to determine whether the associations between socioeconomic status and late-life disability differ by gender in a multi-sociocultural sample from different countries. A cross-sectional study was developed with 1362 older adults from The International Mobility in Aging Study. Late-life disability was measured through the disability component of the Late-Life Function Disability Instrument. Level of education, income sufficiency and lifelong occupation were used as indicators of SES. The results indicated that a low education level ß = -3.11 [95% CI -4.70; -1.53] and manual occupation ß = -1.79 [95% -3.40; -0.18] were associated with frequency decrease for men, while insufficient income ß = -3.55 [95% CI -5.57; -1.52] and manual occupation ß = -2.25 [95% CI -3.89; -0.61] played a negative role in frequency for women. For both men ß = -2.39 [95% -4.68; -0.10] and women ß = -3.39 [95% -5.77; -1.02], insufficient income was the only factor associated with greater perceived limitation during life tasks. This study suggested that men and women had different late-life disability experiences. For men, occupation and education were associated with a decrease in the frequency of participation, while for women this was associated with income and occupation. Income was associated with perceived limitation during daily life tasks for both genders.

RESUMO

OBJECTIVE: This study aimed to assess the prevalence and factors associated with primary dysmenorrhea in a sample of adult women. METHODS: A cross-sectional study was carried out with women aged between 19 and 49 years from a city of northeastern Brazil. Sociodemographic, gynecological, and obstetric variables were assessed by questionnaires and interviews. Dysmenorrhea was measured by self-report, and the Numerical Pain Rating Scale measured the intensity of pain. Statistical analyses included χ2 test, ANOVA, and logistic regression. RESULTS: The average age was 33.2±9.1 years and the prevalence of primary dysmenorrhea was 56% for the whole sample. The average duration of symptoms was 2.7±1.8 days and the mean intensity was 6.1±2.6. The previous cesarean section was associated with a higher rate of primary dysmenorrhea (PR=2.33; 95%CI 1.11-4.90) when considering the whole sample. Women who aged 25-39 years and are insufficiently active had higher rates of primary dysmenorrhea (PR=5.24; 95%CI 1.08-27.31). CONCLUSION: Primary dysmenorrhea has a high prevalence in young adults, adults, and middle-aged women. Cesarean section and being physically inactive was associated with increased rates of dysmenorrhea among adult women.

RESUMO

Abstract Introduction: Physical exercise may improve motor skills, such as static standing balance. However, the association between physical activity level based on activities of daily living and static balance is unknown. Objective: To assess the association between the physical activity level and static balance in middle-aged and older women. Methods: This cross-sectional study involved 589 community-dwelling women. Static balance was assessed using the single-leg stance test (SLST) with eyes open and closed. Physical activity level was assessed using the International Physical Activity Questionnaire Short Form and classified as high, moderate, or low. Kruskal-Wallis test compared balance performance between participants with different physical activity levels. Multiple quantile regression analyses assessed the association between variables adjusted for age, family income, educational level, body mass index, comorbidities, and parity. Results: Participants with low physical activity level showed worse SLST performance with eyes open and closed than participants with high physical activity level in the bivariate analysis. However, physical activity level and SLST performance were not associated in the analysis adjusted for covariates. Conclusion: Our results suggested that only being active in daily living activities is not associated with better standing balance in middle-aged and older women. Specific physical exercise programs should be implemented to improve balance in this population.

OBJECTIVE: To analyze whether maternal age at first pregnancy and parity are mediators of the association between early menarche and metabolic syndrome in a sample of middle-aged and older women. METHODS: Cross-sectional study of 428 women (40 to 80 y), who had experienced a pregnancy in their lifetime, was performed between 2014 and 2016. Age at first pregnancy, parity, and early menarche were self-reported. Metabolic syndrome was assessed using the criteria described by the National Cholesterol Education Program's Adult Treatment Panel III. The association between metabolic syndrome and early menarche was assessed by logistic regression analysis. The mediating role of age at first pregnancy and multiparity in the relationship between early menarche and metabolic syndrome was assessed through mediation analysis, adjusted for covariates. RESULTS: According to adjusted logistic regression models, early menarche was associated with higher odds of prevalent metabolic syndrome (OR: 2.26; 95% CI: 1.15-4.46). Mediation analysis showed a significant direct effect of early menarche on metabolic syndrome (ß: 0.808; 95% CI: 0.107-1.508). Of the two mediators tested, age at first pregnancy was significant (ß: 0.065; 95% CI: 0.004-0.221), ie, participants with and without early menarche differ, on average, by 0.879 SDs in the log odds of MetS (total effect), of which 0.065 SDs (8%), on average, would be attributable to the effect of early menarche on age at first pregnancy (indirect effect), which, in turn, affects MetS. CONCLUSIONS: Age at first pregnancy may partially contribute to the association between early menarche and metabolic syndrome among middle-aged and older women who had experienced a pregnancy over their lifetime.

RESUMO

BACKGROUND: Physical health is an important factor for what is considered successful aging. Using valid and reproducible tools to classify PH of older adults may help to develop appropriate rehabilitation protocols for this population. OBJECTIVE: To evaluate the convergent validity and reproducibility of the International Classification of Functioning, Disability and Health (ICF) core set to classify the physical health of older adults. METHODS: A total of 101 older adults were evaluated for handgrip strength, physical performance (Short Physical Performance Battery), and physical activity level (International Physical Activity Questionnaire). Physical health was classified with the ICF core set (14 categories of body functions, 4 of body structures, 9 of activity and participation, and 3 environmental factors) and an impairment index was calculated for each component. RESULTS: Higher levels of physical activity were associated to lower impairment index in the body function and activity and participation components, but was not associated to environmental factors. Better physical performance and handgrip strength were also related to lower impairment index in all components. The Intraclass Correlation Coefficient analysis indicated good reproducibility for body function, body structure, the capacity component of the activity and participation, and for two environmental factors (use of medications and assistive devices), but moderate reproducibility for the performance component of activity and participation, and poor reproducibility for the environmental factor related to access to health services. CONCLUSION: The ICF core set for the physical health of older adults is a valid and reproducible tool and can be used in clinical practice and research.

RESUMO

BACKGROUND: Intestinal constipation is characterized by problems related to evacuation, and presents high prevalence in the female gender. This condition has demonstrated negative effects on the development of daily activities, causing damage to the physical and emotional well-being of individuals who are diagnosed with it. Studies that investigate what health impairments intestinal constipation can cause are scarce in the literature. OBJECTIVE: This study aimed to verify the prevalence and factors associated with intestinal constipation in premenopausal women living in Northeastern Brazil. METHODS: It is a cross-sectional study. This was carried out in the northeast of Brazil. Participated 195 women, adult and middle age. Social conditions, habits and lifestyle, clinical aspects and obstetric history were investigated. Constipation was diagnosed using the Rome III Criteria. Multivariate analysis was conducted using Poisson Regression with robust variance to analyze the relationship between intestinal constipation and independent variables. A statistical significance level of P<0.05 was considered. RESULTS: Most of the women were between 25 and 39 years old (49.2%) and had an income of up to one minimum wage (79.5%). The intestinal constipation prevalence was 35.4%. In the final multivariate regression model, hemorrhoid clinical aspects (P=0.01), pain (P=0.001) and a burning sensation (P=0.01) on bowel movement, and sexual dysfunction (P=0.03) remained associated with constipation. CONCLUSION: The present study found a significant prevalence of constipation among premenopausal women and clinical factors such as hemorrhoids, pain and a burning sensation, and sexual dysfunction were associated with intestinal constipation.

MÉTODOS: Estudo transversal realizado no Nordeste do Brasil.

The hormonal changes in climacteric women may affect the vestibular system; however, it is not clear in the literature whether the presence of vestibular dysfunction associated with climacteric is related to poorer quality of life. The study sample was composed of 374 women (40-65 years). Socioeconomic and demographic data, menopausal status, practice of physical exercises, presence or absence of vestibular dysfunction, hypertension and diabetes, anthropometric measurements and quality of life (using the Utian Quality of Life Scale - UQoL) were collected. Statistical analyses were performed using the Pearson test, Anova, T-test, and multiple regression considering a significance level of 5%. A significant relationship was found between vestibular dysfunction and health (p = 0.02) and emotional (p = 0.01) domains of the UQoL. In addition, physical activity, menopausal status, body mass index (BMI), waist-hip ratio (WHR), household income and diastolic blood pressure (DBP) mean also remained significantly related to quality of life. A relationship between vestibular dysfunction and quality of life for health and emotional domains in climacteric women was observed.

Abstract Objective To verify the ability to evaluate physical performance tests in the identification of low muscle mass (MM) in middle-aged and old women. Method This cross-sectional study was carried out with 540 middle-aged (40-59 years old) and old women (≥60 years old) in the municipalities of Parnamirim and Santa Cruz in the state of Rio Grande do Norte, Brazil. MM, handgrip strength, knee extension, and gait speed were evaluated. Low MM was defined by skeletal muscle mass index lower than the lower 20% for each age group. The following tests were used: Student's t, chi-square, ROC curve analysis to calculate the area under the curve, and cutoff point of each test in the discrimination of participants with low MM. P<0.05 and CI of 95% were considered. Results For the middle-aged group, the handgrip and knee extension strength showed moderate sensitivity (71.6% and 72.5%, respectively) and specificity (59.4% and 56.0%, respectively) in the identification of low MM. For the old women, gait speed and handgrip strength showed good sensitivity (77.8% and 81.6%, respectively) and moderate specificity (51.4% and 64.5%, respectively). The discriminatory capacity of gait speed for middle-aged women and knee extension strength for old women were unsatisfactory. Conclusion Muscle strength measurements are useful for low MM screening in middle-aged women, while handgrip strength and gait speed tests are useful for older women.

RESUMO

Objective: The objective of this study is to evaluate the influence of urinary incontinence (UI) on physical performance. Method: In prospective analyses from the International Mobility in Aging Study (IMIAS), 915 women (65-74 years) from Canada, Colombia, Albania, and Brazil were evaluated in relation to self-reported UI (past week) and physical performance (Short Physical Performance Battery [SPPB]), with reevaluation after 2 years. Linear mixed models examined the influence of UI on SPPB, adjusted by covariates (age, study site, education, income sufficiency, body mass index [BMI] and parity). Results: Women reporting some UI presented lower SPPB mean (ß = -0.41, p = .009) and a greater reduction (ß = -0.53, p = .001) over 2 years than those reporting no UI. Discussion: Compared with no reported UI, some UI was associated with worse and more pronounced declines in physical performance over 2 years. This study highlights the importance of practices to reduce UI to contribute to healthier aging.

RESUMO

Abstract Objective: to evaluate the relationship between sarcopenia, functional capacity and nutritional status among elderly women living in the community. Method: an observational, cross-sectional study was performed with 100 elderly women aged over 60 years. A questionnaire containing identification and socioeconomic data and information relating to the practice of physical activity was applied, while anthropometric and body composition data were measured through bioimpedance and functionality data was assessed using the six-minute walk test (6MWT). Independent t-tests were performed for the quantitative variables and analysis of variance (ANOVA) was used to compare the means of the variables. Multiple linear regression analyzes were performed to estimate the mean 6MWTfor each of the variables studied. Results: The average age of the elderly women was 67 (± 8.0) years, 41% practiced physical activity, 38% had at least an elementary school education, 48% received up to two minimum wages and 91% were obese according to waist circumference (WC). The prevalence rates for the presence of sarcopenia were: 5% for sarcopenic obesity (SO), 63% for obesity, 14% for sarcopenia and 18% had adequate weight. Elderly women who practiced physical activity, had at least an elementary education and who were non-obese according to WC, performed better in the 6MWT. There were no significant differences in the 6MWTbased on income or Body Mass Index (p>0.05). Non-obese and non-sarcopenic women walked further in the functional test than the other women (p = 0.021). Conclusion: SO was present in 5% of the elderly women and is related to poor physical performance, which was also present in elderly women with sarcopenia and obesity.

Abstract Introduction: The low back pain is a major public health problem in industrialized societies, commonly affecting the adult population. The occupation is considered as a risk factor, especially those functions that require physical effort, we have as example the job of the military police, who are exposed to several risk factors, both those who perform administrative activities, and those who perform the ostensive policing. Objective: The aim of this study was to evaluate the prevalence of low back pain among military police, as well the presence of disability and associated factors. Methods: This is an analytical cross-sectional study. Was evaluated the level of disability, pain before and after the work shift, flexibility, postural alterations and anthropometric measures. It was used the regression analysis, considering 95% CI and p < 0.05. Were interviewed 97 military police with mean age of 39.30 (± 5.33), all males. Results: The complaint of low back pain was significantly larger after the work shift, and 82.5% reported difficulties in daily life activities due to low back pain. Conclusion: There is a high prevalence of low back pain and impairment in daily life activities because of military police. The data presented reinforce the need to consider low back pain as a public health problem that affects the whole population, especially the military police.

Abstract Objective: to analyze the relationship between handgrip strength and lower limb strength and the amount of segmental skeletal muscle mass in middle-aged and elderly women. Methods: an observational, cross-sectional, observational study of 540 women aged between 40 and 80 years in the cities of Parnamirim and Santa Cruz, Rio Grande do Norte, was performed. Sociodemographic data, anthropometric measurements, handgrip dynamometry, knee flexors and extensors of the dominant limbs, as well as the segmental muscle mass of the limbs were evaluated. Data were analyzed using Student's t-Test, Chi-square test, Effect Size and Pearson's Correlation (CI 95%). Results: there were statistically significant weak and moderate correlations between handgrip strength and upper limb muscle mass, knee flexion strength and lower limb muscle mass, and between knee extension strength and lower limb muscle mass for the age groups 40-59 years and 60 years or more (p<0.05). Conclusions: muscle strength correlates with skeletal muscle mass. It could therefore be an indicator of the decrease in strength. It is not the only such indicator, however, as correlations were weak and moderate, which suggests the need for more studies on this theme to elucidate which components may also influence the loss of strength with aging.

OBJECTIVE: In Brazil, information about the influence of body image on the various life domains of women in menopausal transition is scarce. Thus, the objective of the study was to analyze the relationship between body image and quality of life in middle-aged Brazilian women. METHODS: This was a cross-sectional study of 250 women between 40 and 65 years old, living in Parnamirim/RN, Brazil, who were evaluated in relation to body image and quality of life. For body image, women were classified as: dissatisfied due to low weight, satisfied (with their body weight) and dissatisfied due to being overweight. Quality of life was assessed through a questionnaire in which higher values indicate higher quality of life. Multiple linear regression was performed to analyze the relationship between body image and quality of life, adjusted for covariates that presented p<0.20 in the bivariate analysis. RESULTS: The average age was 52.1 (± 5.6) years, 82% of the women reported being dissatisfied due to being overweight, and 4.4% were dissatisfied due to having low weight. After multiple linear regression analyzes, body image remained associated with health (p<0.001), emotional (p = 0.016), and sexual (p = 0.048) domains of quality of life, as well as total score of the questionnaire (p<0.001). CONCLUSION: Women who reported being dissatisfied with their body image due to having low weight or overweight had worse quality of life in comparison to those who were satisfied (with their body weight).

RESUMO

Abstract Introduction Some studies have investigated the influence of hormonal deficits and menopausal status in muscle disorders of women. However, it has not been investigated the relationship of both climacteric symptoms and the perception of quality of life with physical performance. Objective To evaluate the correlation of menopausal symptoms and quality of life with physical performance in middle-aged women. Methods This cross-sectional study was performed from April to November 2013 in the municipality of Parnamirim, in the Brazilian state, Rio Grande do Norte. The sample was composed of 497 women aged 40-65 years. The Menopause Rating Scale (MRS) and the Utian Quality of Life (UQOL) questionnaire were used to evaluate menopausal symptoms and quality of life respectively. Measures of physical performance included handgrip strength, knee extensor and flexor strengths (using an isometric dynamometer), gait speed, and chair stand test. The correlation between menopausal symptoms and quality of life with physical performance was assessed by Pearson's correlation coefficient with significance set at p< 0.05 and a confidence interval of 95 %. Results There was a significant negative correlation between handgrip strength and somatic MRS score (p= 0.002) and total MRS score (p= 0.03). There was a significant correlation between knee flexor strength and sit-to-stand time and all menopausal symptom areas (p< 0.05), except psychological symptoms. There was a positive correlation between physical performance of the knee flexors and quality of life items including occupational (p= 0.001), emotional (p= 0.005), and total UQOL (p= 0.01) , but a negative correlation with sit-to-stand time and all quality of life domains (p< 0.05). Conclusion A greater intensity of menopausal symptoms and worse quality of life were related with worse physical performance. Thus, preventive measures should be implemented to avoid adverse effects on physical performance at more advanced ages.

Objective To investigate somatosensory deficits in the ipsilesional wrist and hand in chronic stroke patients and correlate these deficits with contralesional sensorimotor dysfunctions, functional testing, laterality and handedness. Methods Fifty subjects (twenty-two healthy volunteers and twenty-eight stroke patients) underwent evaluation with Semmes-Weinstein monofilaments, the sensory and motor Fugl-Meyer Assessment, the Nottingham Sensory Assessment in both wrists and hands and functional tests. Results Twenty-five patients had sensory changes in the wrist and hand contralateral to the stroke, and eighteen patients (64%) had sensory deficits in the ipsilesional wrist and hand. The most significant ipsilesional sensory loss was observed in the left-handed patients. We found that the patients with brain damage in the right hemisphere had better scores for ipsilesional tactile sensation. Conclusions A reduction in ipsilesional conscious proprioception, tactile or thermal sensation was found in stroke subjects. Right hemisphere damage and right-handed subjects had better scores in ipsilesional tactile sensation.

Resultados Vinte e cinco pacientes apresentaram alterações sensoriais no punho e mão contralateral ao AVE, e dezoito pacientes (64%) apresentaram déficits sensoriais no punho e mão ipsilesional.
